Step 3: Complete the quote form. Determine your SIC (Standard Industrial Classification). Remember to click on your city of work and residence. Also note under Health, Dental and Disability coverage whether you are an individual, a family, an employee + spouse or an employee + child.
Step 4: Hit submit. You may need to clarify answers to one or more questions. The computing process then takes about 5 seconds.
